According to 2010 statistics, 9.3 million individuals in the US underwent a cosmetic procedure last year. This number represents more than double the cosmetic procedures performed in 1997.

Data from the US Census Bureau and the American Board of Medical Specialties, was used to calculate the most vain state as a consequence of the high number of plastic surgeons. The news source KSC News, reports that The District of Columbia, with one plastic surgeon for every 27,351 residents, is the "vainest" state in the union. Other states include Maryland, Florida, New York, California, New Jersey, Utah, Hawaii and Delaware.

The most common non-surgical procedures in 2010 were botulinum toxin type A injections (such as Botox and Dysport), hyaluronic acid injections (such as Juvederm and Restylane), laser hair reduction, laser skin resurfacing (such as ActiveFX, TotalFX and Fraxel) and chemical peels.

The most common surgical procedures included breast augmentation surgery, liposuction, cosmetic eyelid surgery, tummy tucks (abdominoplasty) and breast reduction surgery.

April is Rosacea Awareness Month

Rosacea : What is Rosacea?

Rosacea is an inflammatory condition primarily of the central face, where dysfunctional or hyperactive blood vessels cause blushing/flushing and inflammation of the cheeks, chin, nose, forehead. Chronic inflammation of these areas is associated with papules and pustules similar to acne, and may also lead to skin thickening, especially of the nose.

An important part of the rosacea exam is polarized light evaluation. This simple, painless look into the upper dermis demonstrates the specific location of capillaries or small blood vessels responsible for the symptoms of rosacea. These capillaries are then targeted with the appropriate laser or light device such as the Nd:Yag laser, Intense Pulsed Light (IPL). Topical and oral therapies as well as proper skin protection against inflammation are also necessary for control and remission of symptoms.

There is no known cure for rosacea. The goal is to identify and avoid possible triggers, and thus reduce flare-ups. By keeping a symptom diary to identify the specific triggers you may have, you may be able to see a pattern of what makes your rosacea worse. Use this information to avoid future flare-ups.

Here are some steps that may help:

Avoid sun exposure. Use a broad spectrum, antioxidant sunscreen every day.

Avoid prolonged exertion in hot weather.

Try to reduce stress. Try deep breathing, yoga, or other relaxation techniques.

Limit spicy foods, alcohol, and hot beverages.

Triggers vary from person to person. Other triggers may include wind, hot baths, cold weather, specific skin products, exercise, or other factors.

Cherry Blossoms are blooming, and we had our first full day of sun in the Seattle area yesterday. Today is the first day of Spring. I have overheard many people express relief that Spring is finally here. As happens with the first sunny days in the greater Seattle area, a flurry of people took time out yesterday and today, to walk the dog, run around Green Lake or stroll around the Lake enjoying the sunny day with friends and family.

This March at SkinMD, we have focused on growing lashes and emphasizing your eyes, stressed the importance of daily use of a broad spectrum UVA/UVB sunscreen and proper, twice daily facial skin cleansing. We thank established and new patients, as well as visitors, family and friends who visited the SkinMD booth at the Northwest Women's Show during the first weekend in March.

Fall and Winter Care for your Skin by SkinMD

The first day of fall/autumn is September 22nd, 2009.

When the seasons change, you may notice that your skin changes as well. Oily skin turns a bit more normal, and normal skin may start to feel a bit drier. Acne-prone and rosacea patients may begin to see breakouts and flare-ups.

As the weather turns cold, you may need to change your skin care products to accommodate changes in your skin. At SkinMD, we recommend that you come in for a skin check-up and comprehensive complexion analysis to get you set for healthy, radiant winter skin.

Here are a few tips to keep in mind as winter approaches:

1. Moisturize, Every Single Day

Apply the recommended moisturizer for your skin after cleansing and applying any prescribed serums.

2. Use your Sunscreen Daily

Sunscreen is a DAILY essential in everyone's skin care routine, even in fall and winter. Overcast skies and relative lack of sun does not preclude protecting you skin from UV rays. UV rays penetrate clouds, overcast skies and reflect off water, glass, cement, sand and snow.

Clearly Different: Acne Treatments Optimized

By the time a patient with acne walks through the doors at SkinMD, they have tried and been disappointed with the efficacy of the handful of over the counter lotions, potions and scrubs they tried. Thankfully, today's acne treatment options are such that we can create a customized regimen to assist our patients to achieve the clearer, blemish-free skin they desire. Treatment options include topical and oral medications, as well as laser and light-based treatments.

Which treatment is best?

It really depends on the nature and severity of acne in the individual patient. We also consider how long they have had acne, what type of acne the patient has and if there is a hormonal influence, what they have tried in the past, and what their response has been.

In general, the most effective acne treatments, should include a combination of medications that target the 4 factors that contribute to acne breakouts. These are:

1. sebum production

2. plugging of the follicles

3. P. acnes bacteria

4. inflammation

Goodbye Acne!

There is NO cookie cutter acne treatment regimen. In putting together an effective acne treatment regimen, it is important for our patient to understand that their improvement will not be instant. Water and your skin

First, the curious facts:

1. 75% of Americans are chronically dehydrated.

2. 37% of Americans have a weak thirst mechanism and often mistake thirst for hunger.

3. While 75% of Americans are aware of the rule of thumb about drinking eight, 8 ounces of water daily, only approximately 34% of us drink this amount of water daily.

4. A University of Washington study found that 1 glass of water is sufficient to shut down night-time hunger pangs for 100% of dieters!!

What do you need to know about water and how it benefits your skin?

Your body requires water to survive and function properly. The human body is about 60% water. Your muscles and brain are about 75% water, your blood is 82% water and your lungs are about 90% water. Water provides much needed hydration to our cells. Hydration facilitates removal of toxins, transport of nutrients, protection of our tissues and regulation of body temperature.

Hydration is essential to the proper functioning of the skin as a whole, especially the outermost layer of the skin. When you are dehydrated, the necessary desquamation (shedding) of the stratum corneum does not occur. This leads to skin cell adhesion and impaction, which gives rise to dry, flaky skin.

What should you be doing to keep your body and skin healthy?

1. Until there is definitive evidence to the contrary, drink the recommended 64 ounces of water per day. No, coffee, tea and other caffeinated beverages do not count. Caffeine is a diuretic that makes your body get rid of water. Carbonated beverages are acidic. To offset the acidity of 1 glass of a carbonated beverage, you'll need to drink 32 ounces of water.

For best results, stick to water. If you must, drink 100% fruit juice diluted with an equal amount of water.

2. Apply a moisturizer to you skin after cleansing. All skin types need moisture. It is a myth that oily skin does not need moisture. Adding moisture to oily skin prevents over- production of sebum. Moisture to dry skin adds much needed hydration. Look for formulations that contain urea and glycerin, which have been clinically proven to penetrate into the skin cells, and humectants like hyaluronic acid, that hydrate the stratum corneum.

The FDA has approved Latisse, a new prescription drug from Allergan, the makers of Botox. Latisse will give you long, thick, drop-dead gorgeous lashes that will make you the envy of your friends and family. They may even accuse you of wearing fake eyelashes!

Allergan conducted a clinical trial with about 280 volunteers (don't you wish you were one of them?), half of whom used Latisse daily for 16 weeks. The study results were reviewed by the Food and Drug Administration, which approved the drug in late December. In that study, eyelashes grew 25% longer, 106% thicker and 18% darker.

Unlimited Tanning today, equals brown spots, uneven pigmentation, skin laxity and leathery, older looking skin in 5 to 10 years. By the time you see these signs of the consequences of your sun-bathing or tanning bed spree, you have long forgotten that you did it.

Ultraviolet exposure (UVa and UVb) whether from the sun or tanning bed is responsible for skin damage associated with early aging or photoaging. While much of this damage is cosmetic, some of the effects, such as skin cancer are more serious. Ultraviolet (UV) radiation breaks down the skin's supportive collagen and elastin fibers. Without this support in the deeper layer of the skin or dermis, the skin loses its strength and flexibility. This loss of dermal collagen and elastin leads to the appearance of vertical lines, deep wrinkles, and loose, sagging skin.

The good news is that if you have had a lot of sun damage in the distant or recent past, there are several treatment options available to you to significantly improve and minimize the effects of photoaging.

Q: My skin is looking dull these days. I am in my mid 30's. I want smooth, glowing skin again. What can I do?

SkinMD Answer: There are a handful of options to get the glow back to your skin depending on your skin type and skin color. After proper cleansing, SkinMD recommends a retinol or alpha hydroxy acid product to help with skin cell turn over. Add to this an antioxidant moisturizer and an SPF 30+ sunscreen. Remember that for best protection, your sunscreen should be applied generously in the morning, before UV exposure and re-applied every 2-3 hours throughout the day. Someone recently asked a questions about one of the skin conditions that is plaguing MILLIONS of people in the world; from teens to men and women in their forties and fifties.

"Why isn't there a way to get rid of acne immediately and permanently in this age of technological and biomedical advances?"

My response is, "Excellent question!" As a physician with expertise in skin care and surgery of the head and neck, and a woman who has personally been looking for acne solutions that work for me, I have often wondered about this question as well.

There seems to be more cases of acne than ever before. This may be due, in part, to stress, hormonal shifts and earlier onsets of puberty, hormonal activity among women in their 30's and 40's. With time and exposure to the elements, our skin changes, losing its youthful appearance and we develop brown spots, wrinkles and laxity. For almost 20 years the original UltraPulse has been unchallenged as the gold standard for ablative CO2 resurfacing, the most effective treatment for rejuvenating aging and photo-damaged facial skin. It's Official! BOTOX reverses the aging process.

We have long suspected it and seen it anecdotally in our patients, but now it is official:

Long term use of BOTOX can reverse the aging process by permanently erasing the forehead expression lines and remodeling the skin, by allowing it to 'rest' and renew.

This confirmation is published in the Journal of Cosmetic Dermatology by Dr Patrick Bowler, London, UK. Dr. Bowler studied subjects whose glabellar (11's), forehead and periorbital (crow's feet) facial lines were treated with Botox Cosmetic for over a 7-year period. At the end of the 7-year study, it was found that skin quality continued to improve over time, revealing smoother, enhanced, yet natural look. Effacement (erasure) of non-reducible forehead lines suggested epidermal and dermal remodeling of skin over long-term treatment.

There was no evidence of development of any new wrinkles secondary to Botox use. Subjects continue to report high levels of treatment satisfaction. No safety issues were raised in the study.

Sun safety, aging skin, rejuvenation, SkinMD and you

First of all, let me mention that I am trained as a specialist in Otolaryngology Head and Neck Surgery. In my 15 years in my profession, I have seen A LOT of cancers of the head and neck. These have included 1/2 centimeter squamous cell cancers that after complete removal left a defect the size of a quarter on the forehead, cheek or nose that then needed to be repaired with rotation flaps, and 1/2 centimeter melanomas that required removing a portion of the nose including the skin, cartilages and bone and reconstructing the defect using microvascular reconstruction using muscle flaps and skin grafts.

I was inspired to write this blog in response to ongoing questions that patients, friends and audience members at seminars in which I have spoken, ask about sunscreen use in Western Washington. I am not as surprised as I used to be when patients who came to discuss and undergo rejuvenation treatments to improve aging changes in their skin tell me they do not use sunscreen or that they occasionally or frequently go to a tanning parlor.

In our counseling before, during and after treatments, the providers at SkinMD review and emphasize that the number 1 treatment and protection to ward of aging changes in the skin, and maintain results we have achieved is the DAILY use of a UVA and UVB broad spectrum sunscreen. My mantra is 7/365 sunscreen. We also discuss best times to go out in the sun to play and the use of sun protecting gear. Wide brim hats, long sleeves, long pants, etc

Last week, the EPA (Environmental Protection Agency) singled out our state of Washington for a program in sun-safety education. Why you ask? After all, we do not get as much sun as California or Texas. It is because, unfortunately, Washington has the fifth-highest rate of melanoma, and the seventh-highest death rate from this deadly skin cancer. The melanoma rates among whites were higher in Washington State than among whites nationally.

It is a common misperception that Eastern Washingtonians are responsible for the melanoma rates. Not true. According to the numbers collected by the Washington State Cancer Registry, several Western Washington counties account for the highest melanoma rates. These include Clark, Jefferson, Kitsap, San Juan and Whatcom counties.

What can we learn from this? The daily use of a broad spectrum sunscreen and wearing protective clothing can not be over stated. The reality is that even the best sunscreen does not BLOCK all of the sun's rays from affecting our skin. Most of us do not apply sufficient sunscreen to provide adequate protection from the get go, nor do we cover all sun-exposed skin, or reapply our sunscreen every two hours as recommended.

Scientists have shown that about 10 minutes of incidental sun per day on your arms and legs is sufficient to give you plenty of vitamin D. Please remember that vitamin D is plentiful in our food supply whether you are a traditional or organic consumer. These include natural sources like salmon, tuna, mackerel, fish liver oils, beef liver, cheese, egg yolks and some mushroom varieties. Fortified foods provide most of the vitamin D in the American diet. Milk, cereal flours, some brands of orange juice, yogurt, margarine, and soy milk. Some calcium fortified fruit juices and drinks contain vitamin D as well.

To summarize, when it comes to skin cancer, an ounce....or two of prevention/protection is better than a big scar and years of surveillance or worse death.

The question of the month from SkinMD patients in July has been what to do about hyperpigmentation and restore the skin to a flawless, even appearance. It seems that July has many people thinking about abnormal pigment deposits in the skin (dyschromia).
